The Participant, (herein called the Releasor) (which term includes successors, heirs, executors, administrators and assigns), in consideration of my participation in the Ski and Snowboard Day, hereby:

(a) acknowledges that my participation in this activity is solely my choice and I voluntarily assume any and all related risks concerning or related to my participation.

(b) releases and forever discharges Spinal Cord Injury Ontario, SCIO Committee Members, Directors, Officers, staff, and members, together with all sponsoring and co-sponsoring agencies or individuals, their agents or representatives (hereinafter referred to as the Releasee(s)), from all actions, causes of action, claims and demands of every nature or kind, including claims based on negligence or gross negligence, arising directly or indirectly by reason of my participating in or being in any way connected with the program/activity/event.

(c) agrees not to make any claim or commence or maintain any action or proceeding, against any person or corporation in which a claim could arise against any of the Releasee(s) for contribution or indemnity by a third party under The Negligence Act or otherwise in respect of any occurrence causing me injury or damages.

(d) agrees to hold the Releasee(s) harmless and indemnify them in the event of any claim(s) for damages brought by my dependant(s), pursuant to the Family Law Act (ie spouse, children, parents, grandparents, grandchildren, brothers and sisters).

(e) authorizes that my name, information about me, my spinal cord injury (if any) and my relationship to Spinal Cord Injury Ontario, along with selected photographs/images of me, be released to Spinal Cord Injury Ontario. I authorize Spinal Cord Injury Ontario to use this material for their purposes in any promotional/fundraising materials and on their websites.

(f) agrees that should any provision or aspect of this Waiver of Liability and Release be found to be unenforceable by a Court of law, the remaining provisions of this agreement shall remain in full force and effect.
